Auto Insurance and Worker's Compensation Insurance claims are accepted on Assignment - with Qualified Attorney Representation*

Dr. Lenkewicz, is a preferred provider for the following INSURANCE PLANS*

• HEALTHMATE (you have 12 visits/yr. to use)
• HEALTHMATE 2000 Coast to Coast (you have 12 visits to use)
• HMO Blue of New England (self refer 3 visits, then 9 pre-authorized as administered by BlueChip, after 12 as determined medically necessary by BlueChip)
• BLUECHIP ( 12 visits)
• CLASSIC BLUE (unlimited; between you and the doctor, when YOU feel better)
• Medicare (12 visits, covers spinal manipulation)

• Beacon Mutual Insurance Co. (self refer then as managed by BC/BS of RI through the Worker's Compensation program)
• Other Worker's Compensation insurance-(self refer, then managed by that company)
• UNITED HEALTH PLANS of NEW ENGLAND (UNHP of NE) (up to 20 visits dependent upon contract)

* Most other Major insurances cover chiropractic care and are accepted, please check with our office manager.
* The Doctor reserves the right to refuse to accept assignment with any case when investigation proves necessary.
